Important Information About Medications Administered At School

         

If your student needs medications administered between 8:30 and 3:00 during the school week, please email the nurses office at cstowers@onamia.k12.mn.us or call the school to schedule a time to meet the nurse to drop off medications. Our COVID plan does not allow for unscheduled drops offs inside the school.

                  Medications MUST:

  • Include full name of student
  • Have a full, intact, affixed and legible prescription label on an ORIGINAL bottle. (Please note: pharmacies will provide a second bottle & label for the school, if needed.  Please ask your pharmacy.
  • Over-The-Counter meds must be in the original, unopened bottle or box.
  • Not be expired
  • Be hand delivered by parent or guardian. Students are NOT allowed to carry any medication on to the bus, for any reason.

                                   

Your child’s medical provider must provide a signed Medication Authorization Form to the school in order for it to be given.  Mille Lacs Health System has our required forms on file and at your request, can fax the completed form to us at:  320-532-4359. These forms are also found on our website under this tab:

     *If at all possible, it is always better to try to schedule the medication dosing for before or after school to limit exposing your child to the nurse’s office.
