Facility Use Request Form

For use of the ROC Great Room and Onamia Fitness Center please complete the facilities use request form and return to ROC Coordinator at kbarnett@onamia.k12.mn.us

 

We will process requests on a first come first serve basis with the exception of Onamia Public  School sponsored events and Community Education events. Once your request has been processed and approved, the facility will be reserved and one copy of the facility use contract will be sent to the contact person via email.

All reservations must be submitted and approved at least two weeks prior to the event.
