Allergies: Students with Physician confirmed food allergies need to have a current Special Diet Statement on file at school.  Forms are available online at http://www.onamia.k12.mn.us under the nutrition or at the school office

Airborne allergies – we have returning students and staff with the following life-threatening allergies. Please do NOT send the following to the school:

1. Bananas
2. Watermelon
3. Peanuts and other tree nuts
4. Barbecue Sauce

Please check our school’s Facebook and website for additions as school resumes.

Yearly Health Form: This form needs to be completed every year. Please be thorough when completing the form. Forms are available online under Health & Nutrition
